We accept insurance and offer a sliding scale — both, not either. Most patients pay between $0 and $25 per visit.

Health First Colorado and traditional Medicare are accepted across all of our clinical service lines.

We are in-network with major commercial plans, and we verify your benefits before your first visit so you know your cost up front.

Income-based fees from $0 to a $25 maximum, funded by grants and philanthropy rather than passed on to you.

Specialized Rehabilitation
Physical therapy, occupational therapy, and speech-language pathology for people with complex neurological conditions and physical disabilities. Our clinicians hold advanced certifications in neurologic specialty care.

Monarch Café Program
A café planned as a Community Rehabilitation Program, with partnerships in development with Colorado DVR and the Poudre School District — paid, skill-building work experience for people with disabilities, inside our own facility.

The Transcend Project was built with, not for, the communities we serve. We are in development with partnerships including City of Fort Collins' Adaptive Recreation Opportunities, Poudre School District's Transition Pathways, and Colorado Division of Vocational Rehabilitation.
